Excerpt from The Clinics of John B. Murphy, M.D, Vol. 4: At Mercy Hospital, Chicago; 1915 Dr. Murphy: What is an intestinal fistula? Intern: It is a break in the continuity of the intestinal canal. Dr. Murphy: It is more than that; a diverticulum would answer that definition quite as well. Intern: A fistula communicates with the outside of the body. Dr. Murphy: Do all fistulas communicate with the outside? Intern: They communicate at least with some canal leading to the outside. Dr. Murphy: No. A fistula may equally well communicate with other structures. An intestinal fistula may open at one end into the intestine and at the other on the surface or into the interior of some other organ or viscus. An intestinal fistula may lead up into the pleural cavity. I have seen an intestinal perforation penetrating into the pancreas, producing a pancreatic abscess and ultimately resulting in a fistula connecting the pancreatic duct with the bowel. I have seen an intestinal fistula connecting the bowel with the interior of the liver, where it opened into a liver abscess cavity. It is not so very rare to have a subphrenic abscess form one end of an intestinal fistula. Tell us some of the different varieties of intestinal fistulas. Intern: An artificial anus. Dr. Murphy: An artificial anus is produced surgically. Name some of those that are produced pathologically. Intern: A fecal fistula. Dr. Murphy: That doesn´t mean much more than an intestinal fistula. Intern: One that communicates with the outside.
